Lokale Lyse bei Oxygenator-Problemen unter vv ECMO bei seltenem Anti-Le(b)-Antikörper nach primär kompatibler AB0- Erythrozyten-Transfusion

Zusammenfassung

Im Rahmen einer superinfizierten Covid-Pneumonie bei vorbestehender Asbestose wurde eine vv ECMO etabliert. Nach primär AB0-konformer Erythrozyten-Transfusion kam es im weiteren Verlauf zu einer Leistungsreduktion des Oxygenators mit Verdacht auf Clotting der Membran. Ein Bridging zum Weaning mittels lokaler Lyse durch Alteplase war nicht erfolgreich, weshalb der Tausch des Oxygenators notwendig wurde. Durch weiterführende hämatologische Untersuchungen fanden sich Anti-Le(b)-Antikörper, die zu den plasmatischen Kälte-Antikörpern aus der IgM-Klasse zählen. Der Patient konnte am 18. Tag von der vv ECMO getrennt werden und trug durch die Transfusionsreaktion keinen Schaden davon. Bei unerwarteten Leistungseinschränkungen der vv ECMO nach Erythrozyten-Transfusionen sollte immer an eine Transfusionsreaktion gedacht werden und gegebenenfalls eine weiterführende Testung über das Labor eingeleitet werden.

Abstract

In this case-report, vv ECMO was established in the context of superinfected Covid pneumonia with pre-existing asbestosis. After red cell transfusion, there was a further reduction in the performance of the oxygenator and possible clotting of the membrane. Bridging to weaning using local lysis by alteplase was unsuccessful, necessitating replacement of the oxygenator. Further hematological examinations revealed anti-Le(b) antibodies, which are plasmatic cold antibodies from the IgM class. The patient was able to be removed from vv ECMO on day 18 and suffered no damage from the transfusion reaction. In the event of unexpected performance limitations of the vv ECMO after erythrocyte transfusions, a transfusion reaction should always be considered, and further testing should be initiated.
