Einleitung: Ein unzureichendes Herzzeitvolumen führt postoperativ nicht selten zu einer akuten Niereninsuffizienz mit Anurie. Eine postoperativ länger eingeschränkte Diurese erfordert bei Säuglingen unterstützende Maßnahmen, um eine Überwässerung zu verhindern und die Elimination harnpflichtiger Substanzen zu gewährleisten.
Material und Methoden: Ein acht Tage alter Säugling mit Transposition der großen Arterien und 2,7 kg Körpergewicht wurde durch eine Switch-Operation korrigiert. Nach erfolgreicher Operation kam es am ersten Post-OP-Tag zu einem systemischen inflammatorischen Response-Syndrom (SIRS) mit Low-Cardiac-Output, was die Behandlung mit einem Extracorporeal-Life-Support-System (ECLS) erforderlich machte. Trotz adäquatem Herzzeitvolumen (HZV) unter ECLS-Therapie bestand 8 Stunden danach immer noch eine Anurie mit Hyperhydratation, weshalb die Indikation für eine minimierte Hämofiltration (HF) am ECLS gestellt wurde. Das neuartige HF-System setzte sich aus einem Infusomaten Space P (B. Braun Melsungen), 2 * „Heidelberger Verlängerungen“ (Fresenius Kabi) und einem Hämofilter HPH Junior (Terumo Corp.) mit einer Membranoberfläche von 0,09 m² und 8 ml Priming zusammen. Das insgesamt 28 ml Priming umfassende HF-System wurde im Bypass hinter dem Oxygenator und vor der ECLS-Pumpe eingebaut. Mittels separatem Single-Use-Perfusionssensor und einem Ultrasonic Flowcomputer (Fumedica) wurde der Blutfluss überwacht.
Ergebnisse: Über die Dauer von 87 Stunden erfolgte eine kontrollierte, den hämodynamischen Verhältnissen angepasste Hämofiltration. Insgesamt konnten 2,9 l Ultrafiltrat gewonnen werden. Bei steigenden Retentionswerten erweiterten wir die reine Hämofiltration mit einem zweiten Infusomaten mit 200 ml/h Dialysatfluss im Gegenstromprinzip zu einer Hämodiafiltration (HDF). Während der folgenden 28 Stunden anhaltenden HDF konnten die Retentionswerte effektiv gesenkt werden. Nach verbesserter kardialer Funktion und einsetzender Diurese konnte das ECLS erfolgreich explantiert werden.
Introduction: Inadequate cardiac output not infrequently leads to acute renal failure with anuria postoperatively. Prolonged restricted diuresis postoperatively requires supportive measures in infants to prevent overhydration and ensure the elimination of urinary substances.
Material and Methods: An eight-day-old infant with transposition of the great arteries and 2.7 kg birth weight was corrected by switch surgery. After successful surgery, SIRS with low cardiac output occurred on the first postoperative day, requiring treatment with an extracorporeal life support system (ECLS). Despite adequate CO under ECLS therapy, anuria with hyperhydration still persisted 8 h postoperatively, so the indication for minimized haemofiltration (HF) on ECLS was given. The novel HF system consisted of an Infusomat Space P (B. Braun); 2 * Heidelberger extensions (Fresenius Kabi) and haemofilter HPH Junior (Terumo Corp.) with a membrane surface of 0.09 m² and 8 ml priming. The HF system, holding a total of 28 ml of priming, was installed in the bypass after the oxygenator and before the ECLS pump. The blood flow was monitored by Ultrasonic Flowcomputer with separate Single-Use Perfusion Sensor R (Fumedica).
Results: Controlled haemofiltration adapted to haemodynamic conditions was performed over the duration of 87 h. A total of 2.9 l of ultrafiltrate was obtained. With increasing retention values (creatinine 1.71 mg/dl; urea 84 mg/dl), we extended the pure haemofiltration with a second infusomat with 200 ml/h dialysate flow (Duosol with 4 mmol potassium) in counterflow principle to a haemodiafiltration (HDF). During the following 28 h of continuous HDF, retention values were effectively reduced. After improved cardiac function and onset of diuresis, the ECLS was successfully explanted.
Discussion: Acute postoperative renal failure can be treated by minimized haemofiltration on the paediatric ECLS with effective volume reduction. Extension of HF to haemodiafiltration removes urinary substances in addition to excess tissue water. Adequate volume removal in overhydrated infants with anuria while the ECLS is running is thus methodically possible with greatly reduced priming volume compared to a conventional dialysis device. Additional Sheldon-catheters for dialysis can be dispensed with in this case. Thus, the connection to a minimized haemodiafiltration represents an easy and careful alternative to conventional dialysis methods.
